Job summary

Exciting opportunities are now available to join the Continuing Healthcare team (CHC) in the role of PHB / Care Management Co-ordinator for 37.5 hours a week.

In order to be successful in this role you will need:

Registered Nurse qualification, AHP or Social Work qualification

Relevant experience of working in a complex organisation NHS or Social Care

Experience of working in partnership with service users, carers, service providers and other professionals.

Demonstrate ability to work defined targets and deadlines

Highly developed interpersonal skills and communication skills

Analytical skills

Main duties of the job

This role will be responsible for:

Undertaking care management duties, in order to meet the assessed needs of patients who receive NHS Continuing Care funding, communicating and working in partnership with GPs, social services and other statutory and voluntary organisations including patients and their carers.

Promoting Personal Health Budgets (PHBs) ensuring integration into continuing healthcare and that they are offered where requested to those clients who meet agreed criteria.

Case managing CHC clients ensuring timely reviews are conducted to ensure health and nursing care needs are being met appropriately and to ascertain care packages continue to be effectively commissioned and are cost effective.

To identify the risks presented by the needs of individuals and develop person centred, cost effective care plans, working with specialist health professionals and the patient to manage these risks.
