role delivering high quality medicines optimisation services? A new opportunity
has arisen within NECS Medicines Optimisation team in Hull. We are looking for
an enthusiastic Pharmacy Technician to join our Medicines Optimisation team,
working within Acclaim PCN in Hull to ensure safe and cost-effective use of
medicines. The successful candidate will work as part of a multidisciplinary
team and alongside pharmacy teams in general practice within Acclaim PCN
undertaking structured medication reviews for patients including patients in
care homes amongst other patient facing roles and will be supported by Acclaim
PCN team as well as NECS Medicines Optimisation team.
The role is hybrid working from
one of the GP practices within Acclaim PCN, one of our NECS offices or home.
The successful candidate may be required to travel between GP practices within
Acclaim PCN.
Main duties of the job
This role will be responsible for:
Providing expert pharmaceutical technical
support to medicines optimisation and pharmacy teams in designated locations to
support the implementation of the medicines optimisation work programme.
Undertaking audits of medicines related issues
within general practice
- evaluate results and
produce and present
progress reports to
PCNs, practices and the medicines optimisation team.
Supporting patient reviews and undertake
medicines reconciliation in line with protocols and standard operating
procedures.
Developing and delivering training to PCN and
GP practice staff to support implementation of the medicines optimisation work
programme.

o Answering queries from patients in order to assist them in obtaining the best healthcare
outcomes from their medication
o Developing and delivering training to PCN and GP Practice staff
o Leading improvements to maximise safe, cost effective best practice in prescribing to
improve the quality of patient care
o Providing leadership for medicines optimisation systems across PCNs, supporting
practices with a range of services to get the best value from medicines by encouraging
and implementing Electronic Prescriptions, safe repeat prescribing systems and timely
monitoring and management of high risk medicines
