Job summary

Sedgefield North PCN is seeking to recruit enthusiastic individual(s) to the role of Health & Wellbeing Coach. The successful applicant(s) will join our friendly Integrated Care Team, managed by a dedicated Team Leader, comprising Health & Wellbeing Coaches, Care Coordinators and Social Prescribing Link Workers, who together provide a range of personalized care approaches for our PCN patients.

We are looking for an individual(s) who will pro-actively engage patients using health coaching techniques, to support them to take an active role in their own health and wellbeing. The Health & Wellbeing Coaches play a critical role in empowering and encouraging patients on the benefits of making lifestyle changes.

The Health and Wellbeing Coaches within the PCN Integrated Care team are responsible for delivering both group sessions and one to one sessions for patients, across all the PCN localities to improve patient health outcomes. You will be based with St Andrews Medical Practice, Spennymoor, but the role covers patients of all five practices and services are offered on a rotational basis across the PCN throughout the year. Our current programme includes successful and popular courses for Drop a Jean Size, Chair-based Exercise, Walking Groups and support for pre-Diabetes patients.

Main duties of the job

Playing a health and wellbeing coaching role; teaching and supporting patients/carers to understand and manage their own conditions and maintain an independent lifestyle through health coaching techniques to encourage patient activation

Supporting the development of personalised patient care plans, liaising with the practice team, patient/carer and the Integrated Care Team as appropriate

Playing an active role in MDT meetings as required (regular practice meetings to discuss high risk and / or complex patients) by gathering information and being prepared to update the team on patient progress towards goals etc. (as per their care plan)

Map and connect community activities/ resources at a locality level including supporting the development of a network of community health champions.

Support the delivery of community based public health initiatives such as physical activity, healthy eating and social connectedness.

Build and maintain strong links with the voluntary sector, supporting the voluntary and statutory sector to network and improve partnership working

Support delivery of systematic self-care support plans for those with COPD, Diabetes, Pre-Diabetes, CVD and multiple long-term conditions

Understand the barriers for individuals/groups in accessing support in the community, and use this insight in developing community-based support, working as part of the wider social prescribing model

About us

Sedgefield North Primary Care Network is an innovative, dynamic PCN comprising 5 local GP Practices based in Spennymoor, Ferryhill & Chilton, Sedgefield, and West Cornforth with a population of over 57,000 patients. The practices have been working together as a PCN since 2019, in partnership with local community health providers, mental health services, care homes and community pharmacies.

We are continually seeking new ways to support and improve local healthcare services for our patient population, by working collaboratively with other agencies and the local population. Through investment in new roles the PCN is expanding the range of services that are available to support practices deliver a high-quality care experience to their patients.

The PCN has developed a multi-disciplinary workforce in partnership with its constituent practices and local health services, which includes SPLWs, Care Co-ordinators, Health & Well-being Coaches, Cancer Care Facilitators, Trainee Nursing Associates, Clinical Pharmacists, Pharmacy Technicians, First Contact Physiotherapists, Advanced Nurse Practitioners and Primary Mental Health Nurses.

We are looking for an individual who will play a role in engaging patients and use health coaching techniques to support them to take an active role in their health and wellbeing. The Health and Wellbeing Coach is responsible for delivering group and one to one sessions to improve patient health outcomes.

The coach will play a critical role in engaging patients and encouraging them on the benefits of making changes. This is a new role in our PCN and as such the coach will be able to help shape and develop the role based on their particular passion such as, healthy eating, physical activity, improving mental health etc, using health coaching techniques.

The health and wellbeing coach will work closely with those of low to medium complexity who will usually have 1 or more long term conditions.

The post holder will have a key role in helping to raise the local populations awareness of the support groups and opportunities available to assist them in achieving their health and wellbeing goals.

As a whole, your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long-term conditions.
