Main duties of the job

The way the NHS works is changing; care outside of traditional hospital settings is quickly becoming a much larger part of what the NHS does.

The Urgent Community Response Team (UCR), the rapid response team will be part of this team within Derby City.

UCR is the collective name for services that improve the care for people through delivery of urgent, crisis response care and/or reablement care response.

We will provide a single, integrated service that covers all these types of care.

This will:

  • enable people to live healthy independent lives for as long as possible in their own homes, or the place they call home
  • reduce the need for escalation of care to non-home settings
  • facilitate timely return to their usual place of residence following temporary escalations of care to non-home settings
  • support the collaborative working required to deliver the requirements of the hospital discharge operating model.

Within Derby City we have a Rapid Response Physiotherapy role available where outstanding teamwork and leadership is evident to ensure that we care for our patients in the best possible way.

As a rapid response physiotherapist you will be working with the team to facilitate hospital discharges, prevent admissions to hospital and care homes, and triage patients appropriate for the service.

Our Rapid Response team work together to provide a service which operates a shift pattern between 8.00am and 18.30pm, covering 7 days, including Bank Holidays.

The successful candidates will have high levels of personal and clinical credibility and the ability to instantly form effective working relationships in a range of work settings.

You will have the opportunity to really make a difference in ensuring that quality services are delivered both now and into the future based around our patient needs.

We will also work with you regarding your on-going development and revalidation.
