An exciting job opportunity has become available to join the Macmillan Specialist Palliative Care Team, working in the South West Quadrant of Lincolnshire, covering the area of Grantham, Stamford, Bourne and Deepings. Specific area to be confirmed.
The post holder will be required to work as part of the team to provide specialist advice and support on symptom management to patients and their families, registered to a Lincolnshire GP, to ensure delivery of safe, person centred and effective palliative care.
The post holder will be required to undertake shifts on the Palliative Single Point of Access Line, to provide remote advice to patients and their families for urgent palliative care enquires. The line operates Monday-Friday 8am -8pm and weekends/ Bank Holidays 8am- 4pm currently.
Applicants must be NMC registered Nurses (adult) with experience in palliative and end of life care. Leadership and teaching experience are also essential for this role.
Main duties of the job
- The post holder will be responsible for assessing complex clinical conditions for patients with a confirmed palliative diagnosis in a community setting and formulate personalised, holistic plans of care.
- You will undertake shifts on Palliative Single Point Access Line providing remote support to patients and their families for urgent palliative enquiries,
- You will provide formal education to develop the palliative care skills of multi-disciplinary teams, including pre and post registration students within the Trust.
- You will liaise with other organisations and stakeholders as part of wider Multi Disciplinary Team to ensure safe, effective delivery of patient care.
- You will contribute to the development of clinical standards, policy and audit.
