Job summary
We are looking for someone with the appropriate skills to lead, manage and develop the anticoagulation management service (AMS) team for community and acute sector patients in our catchment area. You will also be required to provide expert pharmaceutical advice and guidance in thrombosis and anticoagulation to the trust as well to the local healthcare community and safety groups.

Responsible for the efficient and effective provision, leadership, management, and development of the Anticoagulation Management Service (AMS) team for community and acute sector BNSSG ICB catchment area patients.
Provision of expert pharmaceutical advice in thrombosis and anticoagulation to North Bristol NHS Trust (NBT) in accordance with Medicine's legislation, the Code of Ethics of the General Pharmaceutical Council, United Kingdom Medicines Information (UKMI) standards and Trust policies and procedures.
Provision of general pharmaceutical duties to the various sections of the pharmacy department providing leadership and supervision for pharmacy staff.
Contribute to the development and implementation of thrombosis policies and guidelines within the trust and the wider healthcare community.
Teaches and supervises less experienced pharmacists, technicians, and students.
Interviews will take place on the 23rd August 2023.
Main duties of the job
- Responsible for the leadership, management, and development of the Anticoagulation Management service to achieve a high standard of efficiency and effectiveness.
- Line manages the anticoagulation management service team including HR duties, for example undertake appraisals, return to work interviews and annual leave approval.
- To develop clinical pathways in the care of anticoagulated patients and for thromboprophylaxis
- To provide expert Pharmacy advice regarding all aspects of anticoagulation, including choice, patient information and dosing to NBT, patients, GPs, Primary Care Network (PCN) staff and BNSSG safety groups
- To determine the appropriate dosing of anticoagulant medication for referred patients to maintain an optimal level of anticoagulation for each individual patient and minimise adverse effects.
- Lead on Trust patient safety initiatives regarding anticoagulation.
- To investigate instances of hospital-acquired thrombosis and determine root causes where applicable in conjunction with NBT patient safety staff.
- Practice as an Independent Prescriber to support the Anticoagulation Management service (AMS) in inpatient and outpatient settings where appropriate.
- Advise on strategies to ensure cost-effective use of resources and provide reports on the use and cost of medicines to the medical division.
- Represent the pharmacy department and be an active member of the trust thrombosis committee.
