An exciting opportunity has arisen for a highly motivated & experienced individual to join our busy, progressive and supportive Paediatric Diabetes Specialist Nurse Team. This is a Band 6 1.0 F/T position.
The Paediatric Diabetes Specialist Nurses are based in the community at the Child Development Centre (CDC) within Scott Business Park.
University Hospitals Plymouth NHS Trust (UHPNT) provides a local service for children and young people across the Plymouth, South Hams, South Devon and East Cornwall area.
The team are employed by UHPNT but are based 'off site' at the Child Development Centre at Scott Business Park, placing us in the heart of the community. This facilitates our remit of work within the community foremost, with only focused aspects of care held within the hospital setting.
The Paediatric Diabetes Nursing team currently cares for 250 children and young people with a diagnosis of either Type 1 or Type 2 Diabetes in the region, utilising various ways of managing their long term condition. This includes injections, insulin pump therapy and continuous glucose monitoring, under the direction of a Specialist MDT including Consultant Paediatrician, Dietitians, Psychologists and Podiatrists.

Main duties of the job
***Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.***

Clinical & Patient Care
- Utilise inpatient education pathways at diagnosis to teach the CYP and their family about diabetes and how they will manage this condition.
- Under supervision, including mentoring and clinical supervision, to gradually take responsibility to provide specialist nursing care and therapeutic advice to our caseload of patients through our duty role which includes support, advice, and signposting.
- Become an effective advocate and educator, which allows CYP, their families and staff to self-manage their diabetes from diagnosis through to transition.
- To act as an expert resource in Paediatric diabetes to provide knowledge and clinical expertise to teach and support CYP, families, and other professionals, including hospital and education settings.
- With supervising, develop individual patient treatment plans, calculate, and adjust insulin doses.
- To establish links with other organisations providing support and information and utilise these additional resources where necessary
