Job summary
Community Links as part of the Inspire North Group is a provider of mental health support services to socially excluded groups working with vulnerable and disadvantaged individuals and families. We deliver a range of creative services in partnership with organisations who share our values across sectors and communities.
People with psychosis, as with all serious mental illness (SMI), are at a significantly increased risk of developing cardiovascular and cardiometabolic disease and illness. As a Physical Health Practitioner working in an Early Intervention in Psychosis Service, you'll support and advise people on a range of issues, such as cardio-metabolic health, diet, exercise and stopping smoking.
Main duties of the job
You will be responsible for organising, delivering and evaluating screening assessments in a variety of settings, giving face to face and remote advice, support and intervention. There are leadership aspects to this job role and you will also set up schemes promoting a healthy lifestyle, run campaigns and implement government initiatives relating to public health and health promotion. There is autonomy, but you will also be required to build strong working relationships with members of the Aspire team and other teams we work with, including GPs, Leeds and York Partnership NHS Foundation Trust and local community groups.

If you do not have these already, there will be an opportunity to train to develop these competencies. There will be an expectation that this post holder will work collaboratively with the wider team at Aspire, and delegate tasks to other staff members to ensure that clients physical health needs are met. The post holder will support junior staff through a supervision process and other Continual Professional Development (CPD) opportunities and attend their own supervision and CPD. Access to a vehicle is required as the post is community-based.

- Able to undertake physical health assessments with reference to the LESTER Tool.
- Ability to perform venepuncture and electrocardiogram (ECG).
- Knowledge of Physical Health comorbidities and the health needs for people with mental health problems; and associated medications.
- Good technical literacy of Microsoft Applications e.g., Word, Excel, PowerPoint, Outlook, Access, and good written communication skills.
- Able to work creatively and collaboratively and positively with clients, carers, other service providers and stakeholders, using open and honest communication.
- Good interpersonal skills including listening and display empathy.
- Able to work as part of a team but also use own initiative within parameters of responsibility.
- Possess good skills in relation to physical health assessment.
- Have access to a car for work purposes and hold a full current UK driving licence.

- Qualification in plain English Second Language e.g. Urdu, Polish, BSL
