Healthy Horley PCN has an exciting opportunity to expand their Primary Care Workforce by employing an additional Paramedic FCP. We would like to build on our existing PCN team as we continue to recognise the value the additional roles bring to our practices and patients. Our aim is to provide exemplary patient care; find innovative solutions in general practice to deliver the best care we can for our patients.
You
will be joining an enthusiastic team of clinicians and administrators. The role
will contribute to improving the quality of care of our patients across the
network of practices.
Our Primary Care Network (PCN) comprises of 4 practices, covering Horley and surrounding areas (Outwood, Salfords and Smallfield) and Caterham. The Practices (Birchwood Medical Practice, Smallfield Surgery, Wayside Surgery and Townhill Medical Practice) have a combined population of 40,000 patients. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix.
Main duties of the job
This is an exciting opportunity for the successful candidate to join the Healthy Horley Primary Care Network (PCN) team in supporting a wide range of patients of all ages. This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England.
Whilst General Practice has some challenging times ahead, this role provides an exciting and innovative opportunity for someone who wants to make a real difference in the communities they serve. You will work autonomously supporting patients across the PCN, providing enhanced clinical assessment and treatment skills to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems.
You will work closely with clinicians from the Practices across Healthy Horley PCN and will play a vital role in increasing capacity for all appointments including home visits.
The successful candidate will be provided with excellent support, the opportunity for development in the role, and will be clinically supervised by a General Practitioner.

Primary duties and areas of responsibility
- To
provide assessment, diagnosis and treatment at first point of contact by
attending to patients according to patients needs either in the Practice or
patients homes.
- Work
with practice teams to ensure the process for referral of urgent on the day
patients is smooth and appropriate patients are referred making full use of the
practitioners skills.
- To
accurately triage and prioritise patients, including in emergency situations,
demonstrating the use of a variety of techniques to elicit the history of an
event/illness, including past medical and drug history.
- To
evaluate clinical information from examination and history taking and initiate
appropriate treatment and/or referral.
- To
undertake and interpret diagnostics and take appropriate action to manage
patient care, whilst distinguishing between abnormal and normal findings in
order to relate them to a possible diagnosis. To use broad clinical guidelines
(NICE etc) relating them to individual cases, providing assistance and feedback
to the organisation and others on how these should be interpreted.

- HEE accredited and on FCP National register.
- Working at masters level or towards masters in their clinical pillar of practice.
