Job summary
The post holder is
responsible for the delivery of general practice nursing to the whole practice
population.
The focus of the role is both the delivery of evidence-based practice
for patients presenting with a long-term condition, and the provision of
preventative health care to the practice population.
The role needs to be worked flexibly in line with the practice opening
hours.
As an autonomous practitioner the nurse is responsible for their care
delivered, demonstrating critical thinking and skills in clinical
decision-making in the management of patients.
Will work collaboratively within the general practice team to meet the
needs of patients, support the delivery of the Quality & Outcomes Framework
(QOF) and any other performance monitoring systems, and provide nurse
leadership as required.
Provide home visits where necessary and telephone triage if required
You may be requested to undertake duties/tasks that have not been
identified in this job description these will be discussed prior to undertaking
them.
It is expected that as an autonomous practitioner that you undertake
continuing professional development relevant to your role.
Main duties of the job
Assess, plan, develop, implement and evaluate treatment programmes that
promote health and well-being
Assess, plan, implement and evaluate individual treatment plans for
patients with a known long-term condition
Proactively identify, diagnose and manage treatment plans for patients
at risk of developing a long-term condition as appropriate
Partake in all aspects of the Quality & Outcomes framework (QOF) &
ensuring that the practice achieves maximal points in all the QOF areas.
Taking on responsibility for specific QOF areas & completing all
assigned actions from QOF meetings
Demonstrate sensitive communication styles to ensure patients are fully
informed and consent to treatment
Must be able to attend all Practice & educational meetings.
Communicate with and support patients receiving bad news
Recognise and work within own competence and professional code of conduct
as regulated by the NMC
Produce accurate, contemporaneous and complete records of patient
consultation, consistent with legislation, policies and procedures
Take responsibility for own development learning and performance
including participating in clinical supervision and acting as a positive role
model
Support the development of others to maximise potential
Actively promote the workplace as a learning environment, encouraging
everyone to learn from each other and from external good practice

Sheffield.

We are a friendly
well established three site training practice with a current list size of approximately 13,000 looking to recruit an
enthusiastic and committed practice nurse to complement our 3 GP Partner, 4 GP
and 3 ANP team having nurses, healthcare assistants and care co-ordinators
working on all sites.
The practice has a
very low staff turnover and places a high value on teamwork and mutual support. Our strong emphasis on telephone
consultations and triage, long before COVID, allows us to offer excellent
access with all patients offered access to a clinician on the day that they
contact the practice.
