Job summary

Health inequalities are avoidable, unfair and systematic differences in health between different groups of people. Health inequalities are experienced between different groups of people and are often analysed across four main categories:

Socio-economic factors (for example, income);

Geography (for example, region);

Specific characteristics (for example, ethnicity or sexuality) and

Socially excluded groups (people who are asylum seekers or experiencing homeless).

Across Derby City, one of the biggest problems we face is diversity and inclusion. We have pockets of high deprivation rates, lower life expectancy than the National average and a variation of ethnic backgrounds.

Cultural differences are a big factor relating to the uptake of cancer screening, health checks, the importance of immunisations (flu, Covid-19, baby immunisations etc), and the understanding of accessing and utilising primary care services effectively. Whilst we may not be able to change a patient's cultural views on the above, PCNs and practices can try to engage and promote through patient education and a better understanding of patient cohorts. This can be done through training and education of clinical and non-clinical staff whilst engaging with patient cohorts to have a better understanding of their cultural views, beliefs and what options may be best for the patient and how best to communicate with the patient.

Main duties of the job

  • To be committed to addressing Health Inequalities.

  • To champion the work undertaken in the PCN around Health Inequalities.

  • To lead the Health Inequalities work at a PCN level & support the Health Inequalities Directed Enhanced Service.

  • To be responsible for ensuring community engagement work takes place ensuring aims and objectives are met.

  • To support the PCN team to engage with wider system strategies ensuring that health inequalities are being addressed:

  • National level: to engage and share local approaches to address health inequalities (as well as Membership of the national Health Inequalities Improvement Forum and access to NHS Future Collaborative Platform)

  • System level: to represent the general practice neighbourhood and community partners in the ICS inequalities improvement agenda.

  • Place level: supporting the integration of primary care, secondary and community services, in collaboration with peer organisational HI Leads, whilst driving the change to help mitigate inequality. Engaging with other place- based partnerships.

  • Neighbourhood / Community Level: to build collaborative, sustainable and positive relationships with external partners to develop a shared vision and understanding of the issues and challenges and how to work together to meet the needs of the community ensuring that it is from a population perspective.

Primary Healthcare Derby (PHD) is a newly formed company, owned by the 28 practices within the Derby City area.

We are responsible for delivering the NHS Englands Ageing Well Programme for all residents within Derby City. We are a fast growing team, with a huge drive to expand, improve the care and services we can offer to our patients.

All 5 Derby City PCNs have come together to tackle health inequalities to share ideas, resources, and work collaboratively develop a comprehensive plan which tackles multiple health inequalities for our diverse population

    • To ensure clear communication between each PCN to share experience and learning - further enhancing the approaches for each programme.

    • Support internal and external meetings, representing the PCN to update on the Health Inequalities programme.

    • Spread the learning and capture primary care & community engagement

    • Work with other system partners on a population perspective.

Person Specification

Qualifications

Essential

  • Two (2) or more and GCSEs including English and Mathematics
  • A management qualification or equivalent senior experience in health and wellbeing or other relevant area
  • A demonstrable interest in population health and health inequalities

Experience

Essential

  • Minimum 3 yrs post-qualification experience
  • Experience of effective leadership
  • Experience of successfully initiating and managing health and wellbeing related projects or services
  • Collaborative working and of building relationships across the PCNs
  • Demonstrable track record of managing and monitoring performance, achieving KPIs to meet funded objectives and delivery of quality services
  • Experience and understanding of the dynamics of a diverse group of people and able to diffuse potential areas of conflict
  • Ability to lead and manage a team successfully with a focus on both development and delegation
  • Knowledge of health issues that impact older people and understanding of the NHS and other care provision
  • Ability to be an excellent ambassador for the organisation building meaningful relationships with both existing and new contracts
