Job summary

Have ambition and commitment to provide high quality care to patients, and a passion for clinical development. Working in general practice alongside a team of pharmacists, they will be required to act within professional boundaries and provide support in medicine optimisation and to conduct clinical medication reviews.

Masters qualification is required

Main duties of the job

  • To conduct clinical medication reviews and proactively manage complex polypharmacy patients, and reduce inappropriate prescribing where possible.
  • To take responsibility for the management of chronic diseases within the practice
  • To support the repeat prescription system, and to manage the repeat prescribing reauthorisation process
  • To complete medicines reconciliation on transfer of care which includes hospital discharge summaries, and to liaise with the patient and community pharmacists to ensure a continuous medication supply
  • Provide support to practice staff with regards to prescription/ medication queries.
  • To support the development and implementation of cost-effective, evidence-based prescribing across the ICB
  • Provide clinical leadership in medicines optimisation and quality improvement and to manage some aspects of the quality and outcomes framework and enhanced services
  • Develop and implement cost management strategies to ensure financial balance is achieved and maintained.
  • Will need to be proactively involved in quality improvement and audit as well as managing some aspects of the QoF
  • Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate
  • Involve the patient in decisions about prescribed medicines and supporting adherence as per NICE guidelines.

Job description

Job responsibilities

  • Understanding clinical medication reviews, especially with those who are in high risk groups such as: Frail elderly, Polypharmacy, High risk medication & Reviewing pathology results for patients on regular medication
  • To reconcile medicines following discharge from hospitals or intermediate care and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge and reduce risk of readmission
  • To be involved in multidisciplinary clinical meetings within PCN where appropriate
  • To manage repeat prescription requests, resolving queries where possible within scope of practice. To ensure patients have appropriate monitoring tests in place when required
  • Implement changes to medicines in line with MHRA alerts, product withdrawal or shortage and other local or national guidance
  • Contribute to the provision of out of hours/on call/ extended services for the practice and the patients. These can include patient facing and telephone consultation,
  • Signposting to other services/ or healthcare professionals where appropriate, while working within scope of practice and limits of competency
  • Participate in review and setting of policies relating to medicines management and prescribing and help ensure practices prescribe in accordance with local guidelines and formulary
  • Undertake a proactive role in audit and quality improvement implementing recommendation where appropriate
  • To monitor network practices prescribing against the local health economy and make recommendations for GPs or amend within scope of practice, based on findings
  • To provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate
  • Provide assistance in completing structured medication reviews and completing targets in relation to the investment and impact fund (IIF) scheme
  • To maintain full and complete records of all patients contacts using the appropriate clinical templates and coding
  • To maintain own clinical and professional competence and be responsible for own continuous professional development
  • To identify areas of Clinical Risk at Network level and make recommendations to support the introduction of new working practices that will optimise the quality of prescribing and make more efficient use of network resources
  • Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care
  • To take an active role in development and embedding of the PCNs culture, values and reputation as providers of high quality services
  • To work as a flexible member of the PCN providing support to other team members when necessary

  • Mandatory registration with the General Pharmaceutical Council
  • Independent prescriber or currently working towards qualification
  • Minimum of 3 years post-qualification experience.

Desirable

  • Membership Primary Care Pharmacy Association (PCPA)

  • Clinical diploma
