Park Surgery in Horsham, West
Sussex has an opportunity for a 8 session salaried GP. Located in the town centre with on-site parking available.
We are looking for a dedicated professional to
step into this role and thrive within our large busy practice and team. We can offer a flexible/hybrid model with the option of a term time contract. We encourage and support profession development and training in special interests.

work, being an attractive market town surrounded by beautiful Sussex
countryside.
We are a large practice with 23,000 patients and supporting
infrastructure to match. The team comprises 6 GP Partners, 3 salaried GPs, 3
ANPs, 6 PAs, 3 Clinical Pharmacists and a team of highly experienced Practice Nurses & HCAs. We have an extensive PCN workforce consisting of
Clinical Pharmacists, Paramedics, a Pharmacy technician, Social prescribers, Care
Co-ordinator's, a 3 First Contact Physio and a Physicians Associate.
Main duties of the job
Provide high quality medical
services to patients on a personalised list and other registered patients with
the practice including any administration connected with those services.
Provide a full range of services
in line with the requirements of the GMS Contract and Enhanced Services
Contracts. Refer patients as appropriate and in accordance with local
procedures.
Undertake duties such as
prescription signing, prescribing reviews, telephone advice, triage, and late doctor in accordance with the rota.

relationships within our PCN.
We hold twice
weekly clinical meetings- patient safety, palliative care, prescribing and
Frailty MDT with community Geriatrician.
We are lucky to
have excellent Practice Co-ordinators who liaise with the community nursing
teams, OOH and Nursing Homes and help support our vulnerable population

Primary Responsibilities
The following are the core responsibilities of role.
There may be on occasion, a requirement to carry out other tasks; this will be
dependent upon factors such as workload and staffing levels:
a. The delivery of highly effective medical care to the entitled
population
b. The provision of services commensurate with the GMS contract
c. Generic prescribing adhering to local and national guidance
d. Effective management of long-term conditions
e. Processing of administration in a timely manner, including
referrals, repeat prescription requests and other associated administrative
tasks
f. On a rotational basis, undertake Late Doctor roles once a
month.
g. On a rotational basis, undertake Extended Hours 2 Hrs a
month.
h. Maintain accurate clinical records in conjunction with good
practice, policy and guidance
i. Identify, implement and embed best practice through clinical
guidelines and the audit process
j. To take responsibility for continuous improvement and quality
initiatives within the practice
k. Accept delegated responsibility for a specific area (or
areas) or the QOF
l. Attend and contribute effectively to practice meetings as
required
m. Commit to self-learning and continuing professional
development
n. Review and adhere to practice protocols and policies at all
times
o. Be the named lead clinician for a care home including
carrying what is required in the Nursing Home LCS

Partner may be requested to:
a. Participate in practice audits as requested by the audit lead
b. Participate in local initiatives to enhance service delivery
and patient care
c. Participate in the review of significant and near-miss events
applying a structured approach i.e. root cause analysis (RCA)
