Job summary
Primarily responsible for attending to
patients on the day. Diagnosing and treating patients with a variety of
undifferentiated illnesses and their continuing medical and nursing needs.
Ensure that clinical practice is safe and remains within the boundaries of
competence, referring to the GP where appropriate. Based within the practice,
across both sites, also requirements for home visits and Nursing Homes.
Demonstrate safe, clinical decision-making and expert care, acting within
professional boundaries providing care for the patient from history taking,
assessment, diagnosis and treatment including evaluation of care and
prescribing where relevant under the supervision.
Demonstrate critical thinking
in the clinical decision-making process and advanced theoretical and practice
knowledge of advanced practice.
Demonstrate flexibility along with pro-activity
and enthusiasm for delivering excellent patient care. Work with the management
team on delivering strategy both for the practice and ICB.
Main duties of the job
Demonstrate high degree of clinical skill,
competency and multi-disciplinary knowledge. Telephone triage, diagnose, plan,
and evaluate treatment for patients, including prescribing alongside the
on-call doctor.
Diagnose and manage both acute and chronic conditions,
integrating both drug-and non-drug based treatment methods into a management
plan.
Review medication for effectiveness, appropriate to patient needs in
accordance with evidence-based practice. Promote healthy lifestyles, and apply
principles of self-care.
Provide care using advanced practice (including:
non-medical prescribing and supplementary prescribing once qualified); referral
for investigation; and interpreting results.
Work closely as part of the
practice QOF team to ensure current high level of achievement is maintained.

We are large training practice with 11 GPs, 5
Nurses, 4 UCPs, and 5 HCAs caring for approximately 20,000 patients across two
rural sites.
We are a strong, supportive team of 50+ staff
and work closely to provide a high level of service to our patients. As a
training practice, we are used to welcoming and supporting new staff so they
instantly feel at home.

- Ensure collection and maintenance of statistical information required for regular or ad hoc reports and audit
