The Jigsaw/SEND service and Children’s Community Nursing Team are looking to recruit a dynamic and motivated Transition Nurse Specialist to join their team in Southampton.

You will work to support the formal transition of children from 14yrs – 19yrs from child to adult services in partnership with the adult transition team. This will include all elements of transition for children with SEND and Education and Health Care Plans (EHCP) transfers to college and Continuing Health Care (CHC) assessment for care packages. This role will involve close partnership working with multi agency partners, and the coordination of intervention required using specialist knowledge of those young people with conditions likely to extend into their adult life.

As part of the multi-agency team you will be offering assessments and interventions for young people and their families to support them through transition periods and enable the enhancement of skill development as appropriate for the young person. You will be contributing to Jigsaw duty service, providing representation at clinical panels and Forums, and providing a service within Childrens Community Team for a day per week.

You would be joining an established and supportive team which is innovative in its approach to the delivery of high quality and specialist care for young people. You will bring specialist knowledge to the team in terms of learning disability and the needs of young people who present with complex health conditions, including life limiting conditions.

You will be supported with regular clinical/managerial supervision and training opportunities, both in house and external, as is relevant to service delivery priorities. The service is fortunate in its multi-agency arrangement that you will have access to training provide by both Solent NHS Trust as well as Southampton City Council. This truly is a unique opportunity to work within a multi-agency team.
