An exciting opportunity has arisen to be part of the Community Respiratory Team in South Gloucestershire working in partnership with North Bristol Trust providing a specialist integrated pathway for people with COPD and providing assessment and review for people who have been prescribed oxygen. This service enables service users to be supported at home and in the local community thus preventing unnecessary hospital visits and admissions.

We are looking for an enthusiastic and motivated Physiotherapist to work with other experienced nurses and physiotherapists to provide specialist assessment and treatment for people with COPD, ensuring delivery of high quality, evidence based person-centred care.

The pathways which we support are:
• Prevention of Hospital Admission
• Home Oxygen Assessment and Review (clinic based and home visits)
• Pulmonary Rehabilitation
• Early Supported Discharge (under development with NBT to enhance the existing service)

You will provide support and education to enable people with COPD to better understand their condition, improving their health and wellbeing and promoting self-management. You will also work closely with the Team Lead and Clinical Specialists to implement, develop and evaluate this service. You will be required to communicate effectively with all healthcare professionals including respiratory Specialist Consultants at North Bristol Trust.

This post is a permanent appointment for 37.5 hours per week. The service is currently required to work 8.00 – 18.00 over 5 days out of seven days per week. The team will be based at Leap Valley Medical Centre, Downend, Bristol with other Sirona community services. These newly built premises offer office and clinic space as well as onsite parking. The majority of visits will take place in the community and staff will be provided with IT to support remote working.

The successful candidate will have a keen interest in the treatment and management of COPD and understand the impact of this long term condition on the individual and their families.

Regular supervision will be provided by the Clinical Specialists and on-going CPD is actively encouraged and supported. This will include the opportunity to develop skills in capillary and arterial blood gas testing and venepuncture. This post also offers the opportunity to work closely with highly skilled and experienced clinicians in North Bristol Trust.

Pay scales between £26996.00 and £35193.00 pro rata and depending on experience. NHS pensions honoured and available.

Part-time and Jobshare would be considered.
