An exciting opportunity has arisen for a Band 6 Community Learning Disability Nurse within Portsmouth’s Integrated Learning Disability Service (ILDS) hosted by Portsmouth City Council.

As a community nurse within the integrated learning disability team, you will work as a member of the multi-disciplinary specialist health care team, within the integrated learning disability service. Your role will be varied, working with service users, family, and carers that you will establish and maintain positive professional relationships with.

The successful applicant will provide person centred assessments and interventions for adults with Learning Disabilities who have associated complex health needs in partnership with other agencies and carers. As a member of the team the post holder should possess strong interpersonal, communication and literacy skills. Demonstrating an ability to work with a range of professionals, agencies and carers
