Community Sister

We are looking for ambitious and innovative senior nurses to join our teams to help us to continue to provide excellent nursing care within the Portsmouth City Community.

You will provide nursing assessment, care and treatment to adults and older people living at home who have physical health needs which may be complex and long term. You will lead and coordinate a team of qualified practitioners and help to develop junior staff members. You will also work as part of the Senior Clinical team, supporting and supervising other staff members to enable provision of high quality, person-centered care. Using your autonomous advanced clinical skills you will case-manage and plan care interventions for people with multiple and complex long term conditions, with the multi-professional team, with the emphasis of preventing unnecessary hospital admissions.

Your excellent clinical, interpersonal and team skills will enable you to work collaboratively across community, primary, secondary and social care settings. Leadership and management skills are essential for this role. Successful applicants will have good communication and interpersonal skills and will need to be flexible and adaptable. Community experience is desirable although not essential as we have an innovative competency based approach to training and development to support you to maximize your career potential.

In return for your skills we offer regular supervision, internal and external training opportunities and regular contact with other professional networks across the area. You will benefit from experienced and supportive colleagues and be able to develop your skills in a range of areas. The hours of work are 08:30 - 17:00 with weekend working 1 in 3. We are open to accepting both full and part time applicants and can discuss working hours upon interview.
