Portsmouth Rehab and Reablement Team are looking for dynamic and forward thinking community nurses to join our expanding integrated service. Following the principles of homefirst you will assist service users to remain at home through access to rapid support and care, reduce ED attendance and unplanned hospital admission and rapid discharge from the acute hospital – ‘why not home, why not today’.

We welcome applications from committed, motivated and reliable individuals who can demonstrate their ability to provide high quality health, care, support and information to service users and their families. Your excellent clinical, interpersonal and team skills will contribute to the delivery of an integrated community care pathway to enable local people to improve, maintain or prevent a decline in health, supporting self-management of health problems and the maintenance of optimum quality of life and independence.

In return for your skills, we will offer regular supervision, internal and external learning and development opportunities.

The service operates from 7am – 10pm daily with all staff expected to participate in a rotating shift pattern. Applicants will be required to hold a driving licence and have access to appropriate transport due to this being a community post.
