Are you an ambitious, innovative and passionate Nurse, keen to use your skills in order to contribute to the avoidance of admission and responding to plan and deliver care for patients with complex health needs in their own homes? If the answer is YES, then you will become a welcomed and valued member of our dynamic and enthusiastic team within Solent NHS Trust.

You will be based in one of the localities within Portsmouth and work as part of the Health and Social Care Team to avoid admission and support timely discharge. You will work as part of the Senior Clinical Team, supporting and supervising other staff members and enabling the Team, to provide high quality, person-centered care. Your excellent clinical, interpersonal and team skills will enable you to work collaboratively across community, primary, secondary and social care settings.

As a Community Matron you will diagnose, prescribe and implement a range of care interventions for people on your caseload, with an emphasis on avoiding unnecessary hospital admissions. You will be a vital member of the Virtual Ward team, attending weekly meetings and using your autonomous advanced clinical skills to plan care interventions for people with multiple and complex long term conditions, with the multi-professional team. Ideally you will have the Non-Medical Prescriber qualification or working towards this.

In return for your skills we offer regular supervision, internal and external training opportunities and regular contact with other professional networks across the area. You will benefit from experienced and supportive colleagues and be able to develop your skills in a range of areas.
