An exciting opportunity is available for an individual to join the Hafan Deg Ward team. This is based in Ty Siriol Unit, County Hospital. Hafan Deg Ward is a 20 bedded Acute Older Adult Mental Health Unit for males and females over the age of 65 living in the ABUHB area. The ward consists of single bedrooms, some with en-suite facilities. The unit specialists in the assessment, care and treatment of people experiencing a variety of mental health problems. The majority of patients come into the unit on an informal basis (i.e. with their agreement). However, some patients may be admitted under the Mental Health Act. All patients admitted to the unit will be allocated a named nurse who will be responsible for co-ordinating their care whilst they are an inpatient. A team of staff will review the plan of care and agree the future care plan. These reviews take place weekly in ward rounds which the patients will be asked to attend and if they need further assessments by other health professionals or social workers they will be referred as soon as possible. Patients on the unit are actively encouraged to participate in their care. The unit has an active occupational therapy program which provides activities on the unit and in the local community. These activities assist to structure the patient’s time but also enable the staff to assess the patient’s needs and functioning. The Consultant and other health professionals will decide when the patient is ready to leave hospital. The family/carer will be involved in planning discharge to meet the person’s needs. The ward supports John's Campaign and has open visiting hours. This post would enable applicants to access the Journey of Excellence (JoE) professional development programme aligned to the learning and development opportunities offered within the Health Board.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
