For over four decades Countess Mountbatten Hospice Charity have provided outstanding end of lifecare to its community and its services have made a huge impact on many families lives across Hampshire.

The journey towards end of life can be a complex one. Medical advances and changes in family lives mean that the care we provide to our patients and their families constantly evolves to meet the needs and expectations of our Community. Over the last year, Mountbatten Isle of Wight and Countess Mountbatten Hospice Southampton, have developed a unique partnership, with one senior team leading both organisations.

At Countess Mountbatten Hospice we pride ourselves on delivering the highest standard of care and support to patients and their families, and constantly strive to ensure we reach as many people as possible, whenever and wherever they may need us. The hospice serves a population of approximately 600,000 and covers Southampton, and parts of West Hampshire. We are a registered charity and company, with our community at the heart of our services.

Currently we have a 25 bedded In-Patient Unit (IPU), a large day, self-help and rehabilitation centre, a community nursing team, Psychosocial care & Bereavement service and an education team. However, our services and approach to care are being constantly developed and expanded to meet the growing demands of increasingly complex health issues and the needs of the community we serve.

Care at Home Service Lead

Hospice’s have always been part of a ‘Care Revolution’ and this new post has been created in line with Countess Mountbatten Hospice’s strategic direction to develop responsive services for people living in our community and to provide outstanding care for those facing death, dying and bereavement.

Our new innovative five-year strategy will focus on creating new models of care which will concentrate on reaching more people across our catchment area to support and coordinate service provision in our service users preferred place of care. This post is key to every aspect of Countess Mountbatten’s continued success to ensure we develop new and innovative services, which provide excellent clinical care and support to patients and families.

An exciting opportunity has arisen for us to appoint a Care at Home Service Lead to lead, develop and support Domiciliary Care services across Southampton City and West Hampshire. We are seeking to appoint a proactive and highly motivated individual to lead the development and implementation of an evidence based, high quality Fast Track Continuing Health Care Domiciliary Care service based at Countess Mountbatten Hospice.

This role will require a Senior Nurse with inspirational leadership skills, ideally gained through service development and management within a community setting. You will be a proactive nurse who is able to constructively work with the wider multi-professional teams to face the challenges of developing and delivering new services for an ever increasing population.

The successful post holder will provide clinical leadership, development and management to a team of community-based carers who will be delivering care to people at home at the end of their lives. Working closely with Clinical Nurse Specialists, District Nurses and the wider Multi-Disciplinary Team, you will promote an integrated and cohesive team approach which is committed to continuously improving care and outcomes for people who use our services.

This role will be pivotal in shaping the future of the Care at Home Service. You will engage with Commissioners, patients, carers and key local stakeholders in health and social care to improve patient care, promote the philosophy and values of Countess Mountbatten and ensure our services remains central to the delivery of end of life care for the local population.

You will be required to have a clean current driving licence and have the use of a vehicle with the relevant insurance for business in association with your duties as our new Care at Home Service Lead.
