We are looking to recruit a new team of Ward Discharge Facilitators to work on Tintagel Ward. We are looking for motivated people who can support our ward teams in order to facilitate and drive the safe, effective and timely transfer of patient care from the Trust to the most appropriate setting to meet the needs of the patient.

This is an exciting new post in this area; the facilitators will work across seven days of the week. The role will focus on supporting flow of our eldercare patients to ensure all discharges are timely and meet the patients’ needs by liaising with a number of members of the multi- disciplinary teams.

We will consider both full and part time applicants.
