Community Nurse

Job Reference: 249-SCH-S0464

Great Western Hospitals NHS Foundation Trust
Community Nursing Team
£24,214 to £30,112 pa

The Great Western Hospitals NHS Foundation Trust in Swindon is proud to have a team of highly skilled, kind and compassionate staff that deliver high quality care for our patients. The Trust provides acute services from the Great Western Hospital and community healthcare services across Swindon. We are an ambitious and progressive Foundation Trust, committed to continuing our quality journey so that we can exceed our patient’s expectations.

We have recently been successful in our bid for £30 million of national funding to develop an Integrated Front Door, expand our Emergency Department, purchase land for potential expansion and to help us develop new models of care, such as an intensive rehabilitation offer. And we have many more ambitious plans. This is a great and exciting milestone which will help us to maintain quality and safety for people in Swindon and the surrounding areas over the coming years.

Are you a Registered nurse ideally with Community Nursing experience looking for a new challenge in the busy town of Swindon, with easy access to surrounding towns and Villages across Wiltshire and the beautiful countryside in adjoining Counties such as the Cotswolds.

If so come and join one of our four Community Nursing teams Swindon. Within each of our teams we have a Band 7 team leader, at least two Band 6 per team, Band 5, 4 and 3 clinicians.

We provide a range of care and treatment to predominantly housebound patients who often have complex care needs or long term conditions that require treatment and support in wound care, catheter maintenance, palliative care, supporting people to die at home, insulin and diabetes care and a whole range of other health conditions.

We provide a wide variety of flexible working opportunities to further develop your skills and knowledge, working alongside Oxford Brookes University supporting and mentoring student nurses and working alongside our multidisciplinary and community teams which include Community Matrons, Tissue Viability Nurses, Continence Specialist Nurses, ‘Same day’ response team comprising of Nurses and Paramedics, IV specialist team and our Phlebotomy Service and our new End of Life Rapid response team.  We also use the electronic patient record system known as SystmOne .

We have a twilight service until 22.00 hrs and an overnight Community Nursing Service providing care and treatment to patients in their own home environment 24/7. Currently our teams are based in our HQ which is located in North Swindon within a retail shopping area of Swindon, with free parking facilities.  We offer flexible working opportunities to improve work / life balance. There is a minimum requirement to work one weekend in four, however if working weekends is more beneficial around your child care or carers commitments we can accommodate more flexible hours for you. We also require you to work a minimum of two twilight shifts per month, but we can also accommodate these hours around school hours ensuring you can provide the flexibility to commit to one weekend in four and two twilights per month.

We offer staff the opportunity of extending their skills and knowledge in a wide range of clinical and managerial courses and we work closely with our external agencies accessing training opportunities at the local Hospice. We are one of very few trusts in the country that includes a Primary care network and this could enable some opportunities to gain experience in Primary care nursing.

If you are motivated, adaptable and keen to embrace a new challenge and have a clean driving license we would welcome your application. If you would like to have an ‘informal’ visit to learn more about the service we provide and meet our teams we would love to meet you.
