Due to an internal secondment within the Community Physiotherapy Team, an exciting opportunity has arisen to join our Long Term Conditions Support Services on a one year fixed term contract.  This role will provide clinical care for patients referred for community physiotherapy.  Patient assessment and care will be delivered in patients’ homes. Service evaluation and development will be integral to this role.

We are looking for a Band 6 Community Physiotherapist to join our existing team. You will need to be able to work independently, making autonomous clinical decisions and appropriate referrals to other services. 

Central to this role are:

- strong communication skills

- expertise and experience in provision of care for patients with reduced mobility and/or musculoskeletal impairments

- experience of service development and evaluation 

You must be a licensed car driver and have access to a car for this post.
