As part of Dorset’s Integrated Care System journey, six million pounds are being invested in Dorset’s Integrated Community and Primary Care Services. The aim is to develop an integrated workforce within localities, with a strong focus on partnership working. Services will be based upon the needs of people in each locality, and will focus on providing:

  • Services for people with high intensity and complex needs
  • A proactive approach to care for people with medium intensity needs caused by long term health problems

With this new investment, we are looking to employ a Band 7 Advanced Clinical Practitioner to work within the Shaftesbury, Gillingham, Sturminster Newton and Blandford North Dorset Hubs and multidisciplinary teams to provide intermediate care to the population in Locality. We are keen to recruit a practitioner coming from a variety of professional backgrounds with the ability to provide quality and personalised care plan for patients with complex needs. The community hubs work as a multi disciplinary model and consist of Medical Consultants, GP's, Advanced Practitioners, Community Matrons, Medicine Management, Health & Social Care Coordinators, Adult Social Care, etc. Being part of our Hubs in Shaftesbury and Blandford the post will be based within Shaftesbury/Blandford hubs whose primary remit is to prevent unnecessary hospital admissions by supporting individuals to remain in their own homes by providing holistic and generalist assessments at the point of need. You will assist individuals and their carers to proactively anticipate their future health needs, support self-care, develop appropriate plans, manage deterioration via early intervention and work collaboratively with the wider MDT to promote improved outcomes.

This exciting and challenging role offers the post holder the opportunity to provide high quality patient focused care in the community. The role supports the Advanced Practitioner in the community through innovation, clinical skills and diagnosis, treatment, review and referral.

The post holder will be expected to engage and act within a range of services delivered in the hubs including inpatient services, Virtual Wards, rapid access services and care closer to home.

Successful applicants must have a broad experience in community and/or urgent care at a senior level. You must be motivated and enthusiastic and have proven experience of working as an autonomous practitioner.

Ideally applicants will have the following qualifications however if you don’t have all of these but have the right attitude then we can support you to develop further in line with professional advancing practice:

 

Current registration with NMC/HCPC

    • Relevant professional health degree
    • Evidence of recent CPD
    • NMP
    • Post Graduate Diploma in Advanced Practice or specialist practice

The service will cover 8:30 to 18:30 Monday to Friday and will develop and be expanded over time.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check. The DBS levies a charge of £40 and the Trust will recover this cost in instalments from your first 3 salary payments. This will only apply to candidates who are new to the Trust and not internal staff.

Applicants who subscribe to the DBS update service and are able to present a valid DBS certificate will not incur a charge.
