An exciting opportunity has arisen for a Team Lead within our Community Diabetes Specialist Nurse team. The role involves responsibility for the leadership of the team and coordination of its excellent clinical care. The post holder will take a lead role in supporting the development and delivery of clinical standards and the development of integrated diabetes care for patients with diabetes in the central locality of Sussex Community NHS Foundation Trust.

You will be working as an autonomous, expert Community Diabetes Specialist Nurse, delivering evidence–based care and structured education to people with diabetes and their families, as well as other health colleagues, in a community setting.

As the Team Lead Nurse you will also participate in local Diabetes Steering Groups, and work with Commissioners in developing and implementing clinical standards, guidelines and pathways, and improving outcomes for the local population.
