Rapid Response Support Worker

£18,813 - £20,795 per annum pro rata

We require an enthusiastic and motivated individual with excellent communication skills and the appropriate experience to provide care and support to individuals as specified by the Health and Social Care Professionals.

The post holder will be a member of a Rapid Response Team providing a highly flexible rapid response service covering the whole of the Northern locality. They will attend and provide care to service users within their own home environment or appropriate setting, often at short notice and during unsociable hours. They will work with members of the Community Teams in meeting the physical, emotional and social needs of the service user in a way that promotes independence and respects the dignity of individuals.

The successful applicant will need to be able to travel to meet the requirements of the post often at short notice and be expected to work as part of a team and will be required to lone work. They will also be required to be competent at using IT systems as we use electronic patient records (RIO).

The post holder will be based within the community at 20 St Georges Road, Barnstaple, EX32 7AU but the post will cover the whole of the Northern locality. There is a rolling rota of early and late shifts over 7 days.

Our teams of care professionals work with patients and their families to support peoples’ independence, health and wellbeing. We provide support to avoid hospital admissions, and if an admission is necessary, we try to make each patient’s stay in hospital as short and effective as possible before working with them on a safe discharge home.
