An exciting opportunity for a Senior Diabetes Specialist Nurse (DSN) to join our Diabetes Specialist Nursing Team has arisen. The post holder will support the Lead diabetes specialist nurse in service delivery and developments and support of junior members in the team whilst also carrying a complex caseload and autonomous clinical practice.

We have a multi-disciplinary team working within the service including the following members: Consultant, clinical pharmacist, diabetes dietitians, specialist podiatrist, healthcare support worker and admin staff) to deliver a high quality diabetes service to patients. The service sees patients with highly complex diabetes needs, in clinics (across 3 sites) and Homes within the borough of Barnet. The post holder will also be involved in delivering educational packages to patients for better management of their condition. The post holder will be supporting GP practices by telephone/email and through mentoring and participating in case reviews or clinical supervision held jointly with Diabetes Consultant and other HCPs as well as training for healthcare and social care professionals.

We are looking for an individual who is passionate about diabetes care and improving the patient experience and outcomes. Team work and integrated working is a core component of the philosophy of local diabetes care delivery.

You must have excellent communication skills and be able to demonstrate the knowledge, experience and motivation to care for the patients living with diabetes.

The successful candidate will also utilise desirable advanced nursing practice skills and knowledge, in particular, nurse prescribing to meet the highly complex diabetes needs of patients who will be accessing the specialist diabetes services.

Please note that applicants for Barnet post, will need to have access to a car.
