Operational Managers: Surrey Downs Health and Care / Primary Care Networks

As a partnership between the three GP Federations, community trust and acute trust, Surrey Downs Health and Care is delivering true transformational change for adult community services across Surrey Downs to the benefit of patients and the local healthcare system. Our delivery model is formed around a shared vision and innovative use of the wealth and diversity of both the clinical and managerial experience that the partnership brings and as such it will provide a truly holistic, person-centred service that no one partner could provide on its own.

The model focusses on care being provided through distributed clinical leadership using Primary Care Networks (networks of GP practices working together with combined practice populations of around 50,000); integrated community bedded services and specialist services with the development of pathways into and out of the acute trust.

We are looking for enthusiastic individuals who are experienced in operational delivery and seeking new professional challenges to become Primary Care Network Operational Managers. Each GP Federation has direct accountability for the community services within their Primary Care networks. Reporting to the Lead GP / Community Services, the Operational Manager will be responsible for the day to day running of the community services within the identified PCN, including the operational management of clinical and non-clinical staff, responsibility for the financial budget of the community services of the PCN

The individual will need to be able to optimise resource management; minimise boundaries between services, both within and outside the PCN to maximize the impact of integrated care pathways; facilitate effective relationships with and key local stakeholders including local practices, wider partners and the local community.

The successful applicant will have proven management ability and operational planning skills; experienced in collaborative working be able to identify, mitigate and manage risk and take a lead role in supporting the development of the wider PCN ways of working, service transformation and place-based care. All Operational Managers will be expected to work together to ensure achievement of the overall Surrey Downs Health and Care requirements and to participate in matrix ways of working across the wider management structure, in particular with the Director of Community Services and the Partnership Board.

This role has an on-call requirement.
