The South Derbyshire and Dales Adult Community Mental Health Team has a vacancy for an enthusiastic Band 6 Lead Nurse with an interest in recovery, collaborative working and community based practice. We aim to provide a comprehensive, multi professional service that continues to improve and develop services that promote recovery and wellbeing. The team is based in Swadlincote.

This post primarily undertakes the role of Care Co-ordinator for adults who are experiencing severe and enduring mental ill health. The caseload will predominantly be working with adults of working age.

The post involves providing comprehensive evidence based assessment and treatment using a range of appropriate individual and group interventions, based on the local and national practice guidance and underpinned by the Recovery Model. You will need thorough knowledge of the Care Programme Approach and be able to demonstrate expertise in safety planning and manage working autonomously in a community setting. We are looking for experience and commitment together with a desire to make our service the best we can through innovation and skill sharing.

The post holder will need to demonstrate expertise around your relevant professional competencies. You will be expected to be a role model, mentoring and supervising Band 5 staff, so experience of supervision of other staff would be essential. You must have your mentor qualification to be considered for a Band 6 post.

You will need to have excellent communication skills and be able to operate as an integrated member of the multidisciplinary team, whilst maintaining a person centred focus.

The ability to transport yourself and service users across a wide geographical area without compromising service needs is essential.

You will need to be able to work within and part of an experienced, lively and committed team. This is a great development opportunity for a Band 6 nurse to gain experience within a community setting and develop new skills.

To support your continued professional development you will receive regular supervision both individual and group from senior colleagues and management to support you with your caseload.

DBS Check
The cost of undertaking a DBS (Disclosure and Barring Service) disclosure will be met by the individual. The Trust will meet the initial costs of the disclosure which will be deducted from your first month’s salary. The current price of a standard check is £23 and an enhanced check is £40.
