The Practice Consultant post at the Julian Hospital requires someone who can support the Team Manager in the development of a new Social Care team within a multi-disciplinary, multi-agency team setting hosted in NSFT’s site at the Julian Hospital in Norwich. The role is new to the organisation and offers the right candidate the ability to not only help shape the development of the new role, but to be fully engaged within the recruitment and development of a new team within the Norfolk and Waveney STP.

The role will require the ability to provide the specialist knowledge needed to support the management of complex cases and onward patient care for a complex patient group in a sometimes challenging care market.

Additional skills required will include: to support the supervision and development of workers within the team, including promoting NCC, priorities and initiatives to support a strengths-based focus and independence; to support the completion and direction of practice and oversight in safeguarding; to provide consistency to allow the development of a team structure as well as external understanding of the team’s purpose and duties.

You will be required to support the management of case allocation and workflow within the team and provide senior support in the triaging of cases; to support the escalation of cases to a senior level; to provide an understanding of the aims of the services; and assist in discussion with the health services to enable an accurate reporting of data internally and externally. By tackling these challenges, a reduction in the number of delayed transfers of care (DToC) will be an important measure of success by the team, in both improvement of flow and, most importantly, in an improved care journey for those using the service.
