Dynamic motivated clinical pharmacist required in Chiswick Primary Care Network

Are you looking for a Clinical Pharmacist role where you will make a real difference to your patients? Then don’t wait for the deadline to pass - come to join our established clinical team now to deliver a new and exciting service to the patients of Chiswick PCN.

As part of the NHS Long Term plan, the NHSE "Clinical Pharmacists in GP practice” initiative is an exciting element of the integrated care transformation agenda, looking at new ways of working and new models of patient-centred care. We are seeking a highly motivated clinical pharmacist to work alongside other pharmacists to develop medicines optimisation services within Chiswick Primary Care Network which has 8 member practices.

Hounslow & Richmond Community Healthcare NHS Trust will host the role, which will be entirely practice based. The successful applicant will have the opportunity to embed the role of the clinical pharmacist within the PCN. The role will involve working alongside GPs and in varying locations to develop medicines optimisation services.

The main role will include, providing support to GP practices including: Medication Reconciliation, Patient Discharge from Hospital, managing repeat prescriptions with a review of centralising the service, quality and cost effectiveness of prescribing generally. The role is expected to assist the Senior Clinical Pharmacist to develop the medicines management strategy of long-term conditions.

This exciting position requires innovative and visionary pharmacist. The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a number of well-established General Practices. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the new NHS.
