Horsham Primary Care Network (PCN) covers the town of Horsham and its surrounding villages. The PCN team is multidisciplinary, consisting of health, social, mental health practitioners and third sector staff working together to deliver excellent patient care at the heart of the community.

Exciting opportunities are now available for Registered Community Sisters/Charge Nurses and/or District Nurses to take on these exciting and challenging roles from January 2020.

Working within the Horsham PCN team is rewarding and if you are patient-centred, committed to delivering high quality care, and have excellent nursing care skills, a “can do” attitude and a desire to be the best, then this is the job for you.

The post holder will have responsibility for delivering community care to patients including end of life care, will attend health and social care meetings to discuss complex patients at risk of admission, be involved in contingency planning for health, and assist with self-management.

You will be expected to coordinate complex care and assessments and present cases to the weekly MDT. You will also need to work closely with both junior and senior team members and the multidisciplinary team.

The post holder will also be responsible for staff management and development, including appraisals and supervision of junior staff.
