We are looking for a dynamic individual with either commissioning or provider experience to join our CCG Transformation team. We cover end to end pathway redesign from primary care, through to community services (including the voluntary sector) and acute services.

We have a vacancy for one permanent WTE Band 8b Urgent Care & System Resilience Transformation Manager. This role will provide senior leadership to the Urgent Care programmes of work and underpinning projects which make up both the Portsmouth and South Eastern Hampshire Integrated Care Partnership system and the CCGs urgent care transformation programme. The work programme consists of four main areas of focus covering (1) Population Health & Demand (2) Emergency Department processes and Admissions Avoidance (3) Bed Occupancy and (4) Out of Hospital Services. In addition, this role will provide senior leadership for system resilience, ensuring operational resilience can be assured and delivered throughout the year through ensuring basic operational processes are in place, that providers are laying the groundwork for continuous improvement and making sure there is strong governance and system working

You will work directly for the Portsmouth and South Eastern Hampshire CCGs but also within the Integrated Care Partnership to deliver innovation, across the Portsmouth and South East Hampshire system.

Ensuring patients receive the care they need, in the right place and at the right time is the top priority for the Hampshire and Isle of Wight Partnership of Clinical Commissioning Groups (the Partnership).

The Partnership serves a population of more than 1 million people and manages a budget of £1.4 billion. It is made up of five clinical commissioning groups from Fareham and Gosport and South Eastern Hampshire, North East Hampshire and Farnham, North Hampshire and the Isle of Wight.
