An exciting opportunity has arisen for a motivated and forward thinking mental health nurse with a keen interest in the mental health of older people to become part of the Care Homes Liaison Team. If you are someone who wants to make a difference to the lives of older people with mental health conditions living in care homes, as well as for their carers, this is the role for you! Working autonomously and managing a caseload you will apply creative-thinking, role-modelling, education and training on a daily basis.

The teams’ role is to promote mental health and wellbeing of residents in care homes through timely assessments and interventions that include diagnosis, psychological work, medication review, education and training. The ultimate aim is to maintain placements and prevent hospital admission. Bridging the gap between primary care and secondary mental health services, the MHSOP Care Homes Liaison Team enhances the integration between physical, mental health and social care in residential and nursing homes by working collaboratively with providers.

As a Care Homes Liaison Nurse you will establish strong links with the residential and nursing homes within Cardiff and the Vale of Glamorgan and provide expert nursing care, education and training that keeps the older person at the centre of care provision and promotes person-centred and least restrictive care.

Key skills and knowledge; clinical expertise in the mental health needs of older people, education and training, collaborative working.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
