Our physical healthcare services include intermediate care services in Hammersmith & Fulham as part of the Community Independence Service partnership.

Our national, specialist and forensic services include:

  • Medium secure services on the St Bernard’s Hospital site in Ealing
  • Services for people with complex and severe personality disorder at the Cassel Hospital in Richmond
  • High secure services at Broadmoor Hospital in Berkshire

We are a teaching Trust with a growing research portfolio, working with academic and industry partners.

The Intermediate Care team consists of ; community admission avoidance team, Discharge to assess(D2A), admission avoidance beds, therapy beds, neuro beds, End Of Life (EOL) and twilight Nursing.

The Neurological team is based on Rosemary ward which is a 25 bedded geriatrician led ward and patients have access to the onsite re-enablement gym. The ward has a maximum length of stay of six weeks and patients receive specialist therapy input until they are able to return their to preferred discharge destination (where appropriate) with support from our community teams as needed.

The Neurological team want to recruit AHPS with a passion for providing enabling care through a CGA model of care within our level 3 neuro ward. You will need to have an understanding of neurological medicine, Therapy, frailty and the CGA model of care. The team are based within Clayponds Hospital and patients have access to a full MDT and rehabilitation gym.

The Intermediate Care team is passionate about training and education and the successful candidate will be expected to provide teaching on a regular basis and will have the opportunity to receive in-house training and complete CPD courses as appropriate.
