An exciting opportunity has arisen within Newcastle Gateshead Clinical Commissioning Group’s Continuing Healthcare (CHC) Team for an experienced, self-motivated individual who has the relevant skills and expertise for a Nurse Assessor. This role will join our team based within Ridley House in Gosforth; with a possible future move to Riverside House, Newburn. You will work with a cohort of Nurse Assessor’s and will be co-located with the Newcastle Gateshead CCG CHC Team, which consists of Nurse Assessor’s, Case Managers, Administrators and Finance support.


The successful applicant will be a Registered Nurse with current NMC registration and possess demonstrable significant post registration experience.


We are seeking a dedicated, motivated and experienced individual with a high level of emotional intelligence, excellent communication and interpersonal skills and that you able to work to stringent national guidelines. You must be flexible and innovative with excellent organisational skills. You will be required to work with a wide variety of stakeholders, including patients and public, Local GPs, Provider Trusts, Voluntary Organizations, the Local Authorities, Public Health and neighboring CCGs.
CHC is a fast paced environment and the successful candidate with be those able to thrive upon the ability to support clients and their families during difficult and emotive situations.


The post would suit a dynamic individual who is committed to the delivery of excellent healthcare and collaborative working.

This role will require travel across the localities of Newcastle and Gateshead, including home visits
