Our physical healthcare services include intermediate care services in Ealing as part of the Community Independence Service partnership.

Our national, specialist and forensic services include:

  • Medium secure services on the St Bernard’s Hospital site in Ealing
  • Services for people with complex and severe personality disorder at the Cassel Hospital in Richmond
  • High secure services at Broadmoor Hospital in Berkshire

Our new Integrated Care Services in Ealing are looking for a dynamic operational manager with a clinical background to lead and manage the new and developing Ealing Community Partners Single Referral Hub. The Hub opened in October 2019 and aims to ensure that community health services in Ealing are accessed via a single phone number and emailed referrals via a single email address.

The referral hub is in operation 24 hours a day, 7 days a week, and:

  • manages referrals for all the Ealing Community Partners services for adult and children’s services, including urgent and planned care services
  • includes clinical triage
  • provides an information line for patients and/ or referrer queries and concerns
  • receives and coordinates telephone referrals for urgent community nursing needs including rapid response

This is an exciting opportunity for a forward-thinking manager who has had experience of working with people who have urgent health needs, to join a new service and help to shape it to meet the needs of the people of Ealing. You will need to work closely with the Hub clinicians and admin staff managing both urgent and planned referrals ( MSK, specialist nursing, rehab etc) to ensure response times are within the commissioned targets.

You will need to form effective working relationships with all the Ealing community health services as well as key external services, the LAS, NHS 111, GP at home, voluntary sector organisations and other referral agencies.

The post will also provide the opportunity to be part of an energetic and dynamic team who are further developing the model of service as a key part of our ambitious plans to develop integrated services for the people of Ealing.

An interest in data, information management and using technological solutions for referral management would be an advantage.
