General Practice Clinical Pharmacist

Job Reference: 887-EXT-3874HB

External Practice
St Paul’s Network
St Pauls Medical Centre, 121 Swindon Road,
£37,570 - £50,819

An exciting opportunity for a forward thinking motivated Clinical Pharmacist to join the expanding team of Clinical Pharmacists working within the St Paul’s Primary Care Network in the heart of Cheltenham, Gloucestershire.

The Network comprises five practices (Portland Practice, St Catherine’s Surgery, St Georges Surgery, Royal Well surgery and The Corinthian practice) and the PCN benefits from operating at a single site, St Paul’s Medical Centre with a branch surgery in Hatherley.

The successful candidate will work across the five practices developing this new and exciting role closely supported by the GPs, Our Senior Clinical Pharmacist, our existing Clinical Pharmacists and the wider clinical team.

The role will provide a mix of face to face, patient centred consultations and telephone support to patients and external agencies. You will be expected to use your clinical skills to carry out a variety of tasks including medication reviews; reconcile medications at hospital discharge and clinical letters; oversee complex needs of house bound and care home patients and look to review and de-prescribe in polypharmacy.

Ideally suited to an Independent Prescriber with experience in primary care, you will be encouraged to manage suitable long term conditions and use your skills within the clinical multidisciplinary team to advice on medicine optimisation, safety and appropriate prescribing in line with local and national policy.

As a newly formed PCN, the role will suit a forward-thinking, innovative and enthusiastic pharmacist able to work collaboratively across the team and keen to deliver the ethos of the network.

If you are adaptable, self-motivated and organised, possessing the necessary qualities and skills for this post, we look forward to hearing from you.
