Specialised Occupational Therapist, Community Neuro Rehabilitation and Early Supported Discharge for Stroke.

This post provides an exciting opportunity to work in CLCH community neurorehabilitation service in the borough of Westminster. The post holder will be a member of the dynamic multi-disciplinary community neuro team, working with a mixed caseload of clients across 3 core pathways. This will include the Early Supported Stroke Discharge service, the level 2 Specialist Neurological Outreach Rehabilitation Service (in partnership with Imperial) and the Community Neurological Rehabilitation Service. These innovative services provides client-centred intervention for individuals within their homes, clinic settings and the wider community.

Applicants should have experience in the assessment and treatment of stroke and other neurological conditions. You will be responsible for the assessment and management of individuals to promote their independence and support long-term management of their condition.

You will be required to communicate complex needs across a number of agencies and provide training to junior staff. You should have good organisational skills, a commitment to multi-disciplinary working and an understanding of working alongside social services and voluntary agencies.

The post is well supported by a Clinical Lead OT and Team lead with supervision by highly specialist neurological OT within the team. There will be robust opportunities for development of your neuro -rehabilitation skills through regular supervision, in-service training and joint working opportunities.
