Continuing Healthcare (CHC) Nurse Assessor and Case Manager:

The CCG are recruiting CHC Nurse assessors for a mix of full-time and part-time roles’

MKCCG is recruiting a number of resourceful, experienced and confident clinicians to the post of Continuing Healthcare Nurse Assessor and Case Manager. As a Continuing Care Nurse Assessor and Case Manager you will work as part of the Continuing Healthcare team to deliver effective assessments, reviews and case management for clients who may be eligible for NHS funding. You will assess, review and case manage clients in line with Local and National policies and procedures, and ensure that care that you commission meets quality and financial requirements..

The post holder will:

  • Deal with all aspects of NHS continuing care processes to improve services
  • Provide expert clinical leadership and advice to the Continuing Healthcare team and wider health and social economy
  • Ensure that the case management arrangements are in place for the CHC caseload working with specialist teams and clinicians across the local health and care system as appropriate

As the successful candidate you will:

  • Possess a current nursing qualification
  • Have some experience of CHC (Mental Health and/or Learning & Disability) and operating in a politically sensitive environment
  • Work flexibly to support both the senior management team and clinical teams and ensure that learning and good practice is shared across the team to standardise processes and achieve service improvement.
  • Adapt to new ways of working within an evolving environment
  • Be solutions focused
  • Work within an allocated budget to ensure the most cost effective placement option is being commissioned
