About Jesmond Health Partnership

Jesmond Health Partnership is a patient-focussed, forward-thinking medical practice in the popular Newcastle suburb of Jesmond.

We were formed in April 2019, and operate across two sites on Osborne Road in the heart of Jesmond, combining the former practices of The Surgery and Holly Medical Group.

Jesmond Health Partnership comprises 3 GP partners, 5 salaried GPs, an experienced nursing team including Advanced Nurse Practitioners, and knowledgeable, enthusiastic Reception and Admin teams. We are a training practice, use SystmOne and promote digital services, patient engagement and self-care.

Practice Details

  • Good across all areas in last CQC.
  • Significantly better than local or national results for GP Choice feedback.

About the role

We are looking to recruit an individual to work as part of our Acute Care Team. The individual may be an Advanced Nurse Practitioner, Emergency Care Practitioner or a Paramedic Practitioner, preferably with experience of working within Primary Care.

We wish to develop a wider skill mix within our team and look at how we can innovate, work differently, whilst developing a model that works for the benefit of our Patients and Staff. Our current Acute Care team handle the majority of same day aaccess appointments as well as home visiting to housebound patients. We are looking to expand the scope of the team to include proactive management of the housebound / frail population. We are keen to support colleagues developing further skills to carry out this role. As this is a relatively new role, there is scope for an experienced colleague to help shape the development of this model.

About you

We are looking for enthusiastic colleagues to help contribute to this vision and are willing to be flexible for the right candidate. The role(s) will be available immediately and we would welcome discussions with colleagues looking for full or part time rolesand would look to accommodate flexible working.

We would ask that prospective applicants would have the following qualifications and skills

  • Nurse Practitioner or equivalent qualification
  • Confident to work autonomously within a supportive team
  • Independent prescribing (non-medical)
  • Experience in Primary care / Community Nursing
  • Good communication skills

We will be looking to introduce formal and informal peer support across the sites including practice education sessions. We have a history of working innovatively and would welcome colleagues looking to develop their role further, with support, in the future.
