Sheffield is a national trailblazer site for delivering a new place-based community mental health service for adults and older adults. The two-year programme is a partnership between Sheffield Health and Social Care, Primary Care Sheffield and NHS Sheffield CCG and the VCSE. It will focus on developing a primary care and community based support for people with a range of complex mental health issues.

If you are flexible, adaptable and like working in partnership to support people close to their home, this is an exciting opportunity to trial a new network-based service across health and social care. Your work will be aligned with primary care networks and directly linked with a developed voluntary and community sector.

 

Description of the Team/responsibilities/duties of the role:

The successful candidate will work within a new multi-disciplinary team of GPs, primary care, mental health physical health and VCSE.

The new model of working aims to avoid fragmentation and multiple handover between teams. Depending on the population needs and will be a first point of contact for the assessment of need for patients when they present at their GP with issues relating to mental ill health. You will have access to supervision and support and work as part of a wider MDT in this new model of care.

PCS will be the host organization for the purpose of employment but the successful candidates will work directly in the practices aligned to the networks. As an individual you will be keen to influence new models of care and will need to:

  • be a registered professional (RMN )
  • have extensive experience of working within a mental health setting
  • demonstrate skills in assessing and managing people experiencing mental health problems that present to general practice
  • hold a prescribing qualification or be willing to work towards

This is a new role so we are looking for somebody who has the ambition and ability to:

  • Manage their own patient group in line with General Practice procedures and risk management protocols.
  • Provide leadership to the Team in respect of psycho-social interventions to people and carers.
  • Supervise and support other team members
  • Participate in service development and establishing this role
  • Work across neighbourhoods to share best practice and expand the model as and when required
