Southern Health is one of the largest Foundation Trusts in the UK, specialising in mental health, adult and child community health and learning disabilities.


 

We are looking for enthusiastic people to boost our existing Extended Primary Care Team as part of an integrated system working to keep people well at home. Our services cover a wide geographical area including Fareham & Gosport and South East Hampshire. We have two 22.5 hour posts are available with bases in Gosport, Fareham and/or Havant.

The applicant is afforded structured professional supervision and an annual appraisal.

 

These positions are designed to contribute to avoiding hospital admission and use of social care services, facilitating and enabling people to keep well in their own homes. Clients accessing this service will be predominantly (but not exclusively) in the older age group. This post offers the opportunity to work collaboratively across primary and secondary care, Adult Services, the community and voluntary sector.

 

The roles are about coordinating care and support across organisations/teams, referring and signposting people to relevant support to enable them to keep well. Working as part of a team which brings medical and non-medical professions together, this role provides proactive holistic care.

 

The successful candidates would complete a period of induction and will be required to assume responsibility for coordinating a plan of support, working within a scope of practice autonomously. The successful applicants will be expected to lead on support planning and identification of person centred goals. The applicant will need to possess excellent communication skills and be competent in linking with the larger multidisciplinary team across the system.

The successful applicant will be proficient, knowledgeable and competent. They will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of patients, the service and the team.

 

This job involves a proportionate amount of time driving, you will need to be a car driver and have access to a car for work.
