The Northern Care Alliance NHS Group (NCA) unites five local hospitals to deliver high quality care across the North East of Greater Manchester.

Our Care Organisations are designed to operate within our group arrangement of hospitals, community and healthcare services which bring together over 17,000 staff and the services of The Pennine Acute Hospitals NHS Trust and Salford Royal NHS Foundation Trust. The Bury Local Care Organisation was launched on 1 April 2019 with the aim of enabling improvements in population health and wellbeing by providing integrated health and care services to the people of Bury. We are an innovative formal alliance of organisations: Northern Care Alliance NHS Group (Salford Royal and Pennine Acute Hospital NHS Trusts); Pennine Care NHS Foundation Trust; Bury Council; Bury GP Federation; BARDOC; Bury Voluntary, Community and Faith Alliance; Persona Care and Support Ltd.

Background

One of the key ways that health and social care partners are working together in Bury to improve services is the development of the Intermediate Tier services.

The proposed delivery model is designed to bring together health and social workers into a unified Bury-wide integrated team delivering intermediate care services under single leadership.

The key deliverables are to:

  • Introduce home-based care into Bury’s intermediate care offer;
  • Create additional enhanced IMC beds through the dual registration of Killelea House;
  • Bring the existing enhanced IMC beds from Bealey Community Hospital under the intermediate tier offer; and
  • Relaunch Rapid Community Response with a broader multidisciplinary team.

It is also intended that the new model of service delivery should change the current pattern of service demand, use and patient flow by:

  • Focusing on acute admission avoidance by increasing step-up referrals;
  • Achieving higher bed occupancy; and
  • Reducing average length of stay.

We are looking for 2 Pharmacy Technicians to provide support to our Intermediate Care facilities and to work with our Integrated neighbourhood teams.

This is a new service within the Bury area, and would suit a Pharmacy Technician who is keen to be part of a developing role.

You will be based in one of our bed based IMC facilities and will work as part of the multidisciplinary team, to support temporary residents with their medicines, supporting them to live independently on discharge from care. You will assess suitability for self- administration whilst resident in the IMC. You will work with carers and nursing staff, to ensure that patients have sufficient supplies during their stay and on discharge home, liaising with secondary care and the local Pharmacy provider to ensure a smooth transition.

You will provide practical advice on drug administration to support independent living and reduce the need for inappropriate care packages. You will work closely with our Pharmacists, who will accept referrals from the Integrated neighbourhood team and from Rapid Response and may be required to visit patients in their own home to determine how they are managing their medicines.

You will work with other members of the health and social care team for the benefit of the patient.

The successful candidate will need to be able to drive and have access to a car.
