Mersey Care NHS Foundation Trust provides specialist inpatient and community mental health, learning disabilities, addiction services and acquired brain injury services for the people of Liverpool, Sefton and Kirkby, Merseyside. We also provide community physical health services in Liverpool, Sefton, secure mental health services for the North West of England, the West Midlands and Wales and specialist learning disability services across Lancashire, Greater Manchester, Cheshire and Merseyside.

We work closely with service users, carers and partner organisations, we've been able to make real improvements to our diverse range of services, from health and social care, right through to high secure provision. Exciting opportunities have arisen for Band 5 Care Practitioners in the District Nursing Service. The service is undergoing significant developments as part of our plan to deliver integrated community care for patients on a neighbourhood footprint across Liverpool and South Sefton.

Working to the Mersey Care values, you will have excellent empathy, communication skills and a person centred approach. You will be keen to continue learning and develop the competencies needed to give evidence based care to your patients in their own home. Clinical decision making skills and a problem solving approach is an essential component of this role. You will be a good team player who can also work autonomously on your own, escalating and reporting back appropriately.

The service operates 24 hours per day, 365 days per year and there are opportunities to work shifts across days and nights (flexible working may be considered)

This is a dynamic, challenging and rewarding position. You will have the opportunity to be mentored and full support will be given to develop and enhance your skills..
