ADVANCED NURSE PRACTITIONERS (Consultant Practitioner)

Essential Requirements: MSc in Advanced Clinical Care and V300 non-medical Prescribing Qualification

Applications from new qualified Advanced Practitioners will be considered and permanent or secondment opportunities for internal candidates are available.

The Urgent Care Team will enable people to remain in their own homes and prevent unnecessary Accident and Emergency attendances and subsequent non elective admissions by providing a community service for up to 72 hours to support adults with a health or social care crisis at home.

Our Team will provide a 7 day urgent and timely response to referrals from the ambulance service and other health care professionals.

We are a clinically led multi-disciplinary team comprising advanced practitioners, occupational and physiotherapists, nurses, support workers, pharmacists and social workers. We are supported by community and hospital based medical consultants.

Our team aim to support adults with a health or social care crisis to remain in their own home, seeking innovative alternatives to hospitalisation wherever possible.

You will help to lead, shape and develop the service, using your expertise to work in a truly integrated way, and championing our approach to integrated care. As well as bringing your clinical expertise, you will be a strong team player, an excellent communicator, adaptable, committed, well organised and work well under pressure. You must be able to travel around various community locations delivering care.
