Staff Nurse

An opportunity has arisen to work within a dynamic, innovative and friendly team within Wirral Community Health and Social Care NHS Foundation Trust Integrated Single Gateway. The Integrated Single Gateway and Community Rapid Response Service is going through a very exciting period of transformation.

This is an excellent opportunity for an experienced nurse with a passion for high quality care.

We are looking for candidates with strong clinical skills and experience and who are patient and team focused. The successful candidates will demonstrate that they are willing to support innovation, and are committed to service improvement in an extremely interesting, and challenging environment.

You will be working as part of a highly supportive and motivated team with strong clinical guidance and leadership, aiding your own professional development Wirral Community Health and Care NHS Foundation Trust is committed to the development of staff and provides learning opportunities to enhance individual and team opportunities. The successful candidates will also work with nurses, therapists and social workers, who are all co-located, in determining the course of clinical care that is right for the patient. To provide Nursing care for acutely unwell patients in their own home and to avoid unplanned hospital admissions.

To use established pathways and processes to provide a wide range of nursing and Social care assessments to patients who may potentially be at risk of a hospital admission.

To identify, assess and co-ordinate discharge diversion for those patients in ED (emergency department) and acute assessment areas or in the community who require social therapy or nursing input rather than in patient care ensuring identified needs are met.

To be an integral part of the Single Point of Access. To support the telephone triage of patients, working with both healthcare professionals and patients in supporting a positive patient journey.

We welcome applicants from a range of backgrounds including emergency nurses, primary care and acute medicine as well as nurses from community backgrounds who have experience in working within Integrated Services.
