The social care team have a secondment opportunity for 6 months in the Inpatient Social Work team based at the Warneford Hospital, Oxford.

The post holder will work within a designated inpatient area, supporting the adult inpatient wards within that area.

The post holder will ensure that all service users admitted to inpatient wards have timely discharge plans in place from the point of admission. The post holder will identify any barriers to discharge at the earliest possible stage of the admission and work with the service user and Adult Mental Health Team to address these barriers and reduce delays in timely discharge.

The post holder will ensure that service users and carers have an assessment under the Care Act 2014 and will support the provision of services to meet identified eligible needs.

The post holder will provide expertise for inpatient staff with regards to Adult and Children’s Safeguarding responsibilities.

The post holder will provide expertise with regards to application of the Mental Capacity Act.

The post holder will demonstrate the skills to maintain their Social Work professional identity when working within an inpatient environment.
